Kinship Navigator Programming Specialist

Overview

Kinship Navigator Programming Specialist

Licensed Social Worker (LSW) preferred.
Bachelors’ in social services, education, gerontology, or related field preferred, plus 2 years of related experience or Master’s degree in lieu of experience in related field. Licensed Social Worker preferred. To provide information, screening & assessments; resources, benefit counseling/navigation, educational programming for Kinship Clubs; and other services as needed.
Outreach, workshops, and other events in Lucas County. Position requires excellent communication and computer skills, and outcomes focus.
Reliable transportation, insurance, driver’s license and successfully passing criminal background check and drug/alcohol screening required.

Care Manager Assistant-Ohio Home Care Waiver and Specialized Recovery Services Program

Employment Status: Full-time

Overview

Description: Assist assigned Care Managers by handling general telephone inquiries and forwarding calls to appropriate staff; checking and triaging care manager voice mail; sending faxes, creating memos, letters, duplicates, sorts, and collates documents; monitors forms, supplies, office equipment and reports findings to designated staff.
Reliable transportation, insurance, valid driver's license
and successfully passing criminal background check and drug/alcohol screening required.

Qualifications: 2 year degree in social work or human services, health related field, or LPN preferred.

Associate Quality Assurance Coordinator

Employment Status: Full-Time

Overview

To assist with managing the development, procurement, contracting, monitoring, and quality assurance of service providers for Medicaid Waiver programs. To review, analyze, monitor, and report provider quality and program utilization data. To provide education and technical assistance to contracted providers.

Bachelor's degree in planning, business or public administration, social sciences, nursing, or related discipline preferred and three (3) years in Health or Human Services Management. Should possess knowledge of Medicare, Medicaid, quality assurance, utilization review, data analysis techniques, and familiar with third party contracting.

Registered Nurses or Licensed Social Workers with one (1) year experience in-home services, home health, medical social work, or gerontology needed to work in our Toledo office.  This position conducts in-person assessments for individuals in need of community based long-term services and supports. Recovery Care Manager

Employment Status: Full-time

Overview

RN, LSW/LISW or Bachelor’s Degree in Counseling, Psychology, or related field and three (3) years post degree experience in mental health or care management. To coordinate specialized recovery services for persons with mental illness. Provides assessment, care planning, service coordination & referral, and assessing Medicaid services. Some On-Call. Knowledge of community resources required.

Valid driver’s license, insurance, reliable transportation, successfully passing criminal background check, and drug/alcohol screening. Excellent Benefits.

Care Manager with one (1) year experience in home care, or gerontology. To work with with frail older adults and younger persons with disabilities in PASSPORT, Aetna MyCare Ohio, Buckeye MyCare Ohio and Ohio Home Care Waiver and other Home and Community Based Services programs in the Toledo area. To conduct care management, and assessments for individuals in need of community based long-term care. Benefit package. Knowledge of community resources and reliable transportation required.

Salary: $44,000 - $48,000/year

  • home care for elderly, home health care, NF, medical social work or gerontology: 1 year

Required license or certification:
